Dear Chief:
This letter is to inform you that [I, my spouse or child] has been diagnosed with epilepsy. [I, my spouse or child] take [name of medication] to control the epilepsy, however the medication does not guarantee complete control the epilepsy. There may be times when [I, my spouse or child] will have a seizure in our home or in public. This letter is to inform you of the possibility of [my, my spouse's or child's] seizure activity.
[I, my spouse or child's] typically [describe the behavior when having a seizure] when [I, my spouse or child] has a seizure. However, there is no guarantee that the behavior will be the same with each seizure, and the behavior may change with each seizure. Please be advised that if you or your employees are called to our home, or come upon [me, my spouse or child] in public, and are informed that [I, my spouse or child] is having, or had, a seizure, please do not disregard that information when responding to the call.
Finally, I would like to know if your paramedics and EMT’s are trained in identifying, assessing and treating a person having epilepsy and various types of seizure activity. Please respond to this letter with that information and additionally, please provide me with the name of the resource hospital where your paramedics and EMT’s are trained.
